BID PROPOSAL FORM
BID FORM: This proposal shall be executed and submitted in accordance with the specifications. All articles and requirements contained in the Invitation for Bid (IFB) Instructions/Provisions to bidders (01/31/05) and on the Bid Proposal Form shall remain and become a part of the contract for requested. All appropriate blank spaces shall be filled in. BID MUST BE SUBMITTED ON THIS FORM AND MAILED USING THE ENVELOPE SUBMISSION FORMAT ATTACHED, OR SUBMITTED USING THE OFFICIAL ELECTRONIC BID SITE. WE WILL NOT ACCEPT A FAXED BID FOR THIS SOLICITATION!
LOUISIANA PREFERENCE: Any materials, supplies, or equipment manufactured, processed, produced or assembled in Louisiana as defined by LRS 38:2251 and for which you have claimed preference according to the product type, must be identified by bid item number on the BID PROPOSAL FORM with the full name and complete address (location) of the manufacturer, processor, producer or assembler, and the percent of preference being claimed. CPSB may choose to allow a bidder offering a product manufactured, processed, produced or assembled within Louisiana as per LRS 38:2251, whose price is within 10% of the low bid price, to receive the award if the vendor of the Louisiana product is willing to provide it at the low bid price.
DEBARMENT CLAUSE: Prior to completing Caddo Parish School Board bids, please read the following instructions:
(1) The prospective participant certifies, by submission of this proposal, that neither it nor its principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in this transaction by any Federal or State department or agency.
(2) Where the prospective participant is unable to certify any of the statements in this document, such prospective participant shall attach an explanation to this proposal.
INSTRUCTIONS FOR PREPARING BID. Bidders are requested to bid on all items within each group. Bids will be awarded as indicated within the solicitation. The bidder with the lowest acceptable group total meeting specifications will be awarded the business covered therein. Each bidder shall be responsible for the accuracy of all unit prices, extensions and/or group totals. CPSB will assume that each TOTAL NET BID is a correct and legitimate bid.
Groups may consist of one or more than one item. To have a complete bid for a group, it is the responsibility of the bidder to fill in the spaces provided for the unit price, the item extension (quantity times the unit price), and the group total shown as the TOTAL NET BID. Failure to bid on all items within a group or to complete all spaces within a group including the TOTAL NET BID may cause the group to be rejected.
